Is Iontophoresis Safe? What You Need to Know Before Starting

It is completely reasonable to wonder whether passing an electrical current through water is safe before you try it on your hands or feet. The short answer is yes, when it is used correctly and within the appropriate guidelines. Understanding how iontophoresis works helps explain why its safety record is so strong. Here is the fuller picture.

A Well-Established Safety Record

Iontophoresis for hyperhidrosis has been used since the 1940s. Decades of clinical use and research have built a substantial body of evidence supporting its safety profile. It is recognised as a standard treatment option in dermatology and is used in clinics and at home all over the world. It is non-invasive, drug-free, and fully reversible, which is a large part of why it is so well tolerated.

Common Side Effects of Iontophoresis

The most frequent side effects are mild and temporary:

  • A tingling sensation during treatment, which is expected and entirely normal at appropriate current levels.
  • Mild skin redness after a session, which usually settles within 30 to 60 minutes.
  • Occasional skin dryness or slight irritation, particularly with very frequent sessions.
  • A brief, harmless shock sensation if the hands or feet are moved out of the water mid-session. This is a built-in safety alert from the device, not a fault.

Applying a fragrance-free moisturiser after each session helps soothe any dryness, and these effects typically ease as the skin adapts to regular treatment. Starting at the lowest effective current and building gradually over several sessions is the most reliable way to avoid unnecessary irritation.

Who Should Not Use Iontophoresis

There are specific situations where iontophoresis is not appropriate. As set out in the Saalio® instructions for use, treatment should not be performed by people with any of the following:

  • A pacemaker or an implanted cardioverter defibrillator (ICD)
  • Pregnancy
  • A metal-containing intrauterine device (relevant to feet treatment)
  • Metallic implants or other conductive objects in the current path between the electrodes
  • Pathologically altered skin or large skin defects that cannot be covered with petroleum jelly
  • Palpitations or cardiac arrhythmia
  • Neurological conditions such as epilepsy or limited skin sensitivity
  • Malignant disorders in the area being treated
  • Severe local inflammation or thrombosis
  • Severe vascular disorders

There are also age limits: hands and feet treatment is not suitable for children under 6, and underarm treatment is not suitable for children under 12. Special Considerations

A few situations call for extra care even where there is no strict contraindication. People with very sensitive skin or a history of eczema may want to start at the lowest possible current and build up slowly, and may find pulsed current mode gentler than direct current. A fragrance-free moisturiser after each session helps prevent dryness.

For children who meet the age requirements, iontophoresis is safe and effective. Parental supervision is recommended, and starting at lower current levels than an adult would use is good practice. The Saalio® is suitable for hands and feet from age 6, and for underarms from age 12.

Using Iontophoresis Safely

The key to safe treatment is simple: follow the instructions in the device manual and never push through discomfort. The current should be increased gradually to a comfortable tingling sensation, never to the point of pain. Start low and build up across sessions as your tolerance grows. Ordinary tap water is all that is needed, distilled water will not work because it does not conduct the current. Used as directed, with gradual current increases and proper skin preparation, the vast majority of people experience only mild tingling and no significant side effects.
